Herefordshire Primary Care Trust Older Adult Mental Health Service (DMHOP) Occupational Therapists (OT) Band 6 (£24,831 – £33,436)pro rata 26 hours per week We’re offering a great opportunity for you to share your skills within our Community Mental Health Teams for Older Adults. The Older Adult Mental Health Service works with older adults with organic and functional mental illness. Using the framework of the Care Programme Approach you will assess, plan and implement care, acting as care co-ordinator in your own right. By bringing your specific clinical experience, OT perspective and skills to the team, you will enhance the joint assessment process. The multi-disciplinary teams include social work, nursing and medical staff and are based in Hereford City. They link closely with our Day Care and respite provision as well as voluntary agencies. Experienced colleagues within the well respected DMHOP service will support in your role. The PCT wide Occupational Therapy Service offers regular CPD Sessions, excellent peer support and co-ordination plus an Annual Away Day.
