Description :
The team is co-located with a range of health professionals, with an overnight short break unit also on the same site, all providing services to children/young people with disabilities and/or long term illness, and their families.
The team works with children and young people, from birth up to age 18, who have permanent and substantial disabilities. It provides a full range of social work functions, including duty, assessments, care planning, reviews, transitions planning and safeguarding. Most of the children/young people who receive a service from the team live at home a small number are 'in care' or residential schooling; a handful of children are currently on child protection plans. We are looking for a Social Worker with prior experience in undertaking child protection work and/or working with children/young people with disabilities and their families, to compliment the existing team skills mix.
